Direct Primary Care Practice Membership Policy
This Membership Policy outlines the terms and conditions for participation in our Direct Primary Care (DPC) practice. By enrolling in our membership, you agree to the following terms:
1. Membership Fee The membership fee is $50 per month for each member. This fee covers access to our primary care services, including same-day or next-day office visits, with a co-payment required at the time of each visit.
2. Commitment and Cancellation We ask that members commit to a minimum of three (3) months upon enrollment. After the initial three months, membership can be canceled at any time with no additional obligations. Cancellation requests must be submitted in writing, and the membership will terminate at the end of the current billing cycle.
3. Office Visits and Co-Payments Each office visit requires a $20 co-payment at the time of the visit. There is no limit to the number of visits a member may schedule. Office visits are typically available the same day or the next open business day, depending on availability.
4. Additional Costs for Procedures and Products Certain treatments, procedures, and products are not included in the membership fee and will be charged separately. These include, but are not limited to: • Cosmetic medicine treatments • Bioidentical hormone replacement therapy • Intravenous (IV) nutrient infusions • Acupuncture • Minor surgical procedures Prices for these services are competitive and typically lower than what is charged by traditional insurance-based practices.
5. Prescription Medications, Labs, and Imaging Prescription medications, lab tests, and imaging services such as X-rays are not covered by the membership fee. These services will be billed separately by the pharmacy, laboratory, or imaging center and are subject to your individual health insurance plan, if applicable.
6. Insurance While our DPC practice does not bill insurance for services provided under the membership, members may continue to use their insurance for outside services such as prescription medications, laboratory tests, and imaging. The terms of coverage for these services will be determined by the member’s insurance provider.
7. Legal Disclaimer This membership does not constitute insurance and is not intended to replace comprehensive health insurance. Members are encouraged to maintain insurance coverage for hospitalizations, specialist visits, and other medical needs that fall outside the scope of primary care. The DPC membership covers only the services provided by the practice, and the practice shall not be liable for any costs or claims related to services rendered outside of the practice. Acknowledgment of Agreement By enrolling in this DPC membership, the member acknowledges and agrees to the terms outlined in this policy.
This agreement is governed by the laws of North Carolina and may be amended by the practice with notice to the member.
